What you need to bring

On the day of your appointment, arrive 10-15 minutes before your start time. Be prepared to complete some required paperwork and answer some basic questions about your physical and mental health, circumstances, substance use, and your current requirements.

Please have all of the following with you:
• Valid Identification
• Insurance Card (s)
• Referral or Discharge Paperwork
• Medication List
A client handbook will be provided to all clients with various resources and Information upon admission.

Insurance Verification

Regarding insurance, we are in-network with Medicaid, Medicare, and most other private insurance plans accepted by Rutgers University Behavioral Healthcare.
